Combined cyclodialysis and cataract extraction

    Summary

    This study shows that combining cyclodialysis with cataract extraction is a safe and effective treatment for severe glaucoma uncontrolled by medication. The combined procedure significantly lowered intraocular pressure in most patients, improving vision and eye health.

    Background:

    • Severe glaucoma inadequately controlled by maximal medical therapy requires surgical intervention.
    • Cataract extraction is necessary in patients with co-existing glaucoma and visually significant cataracts.
    • Combined surgical procedures aim to address multiple ocular pathologies simultaneously.

    Observation:

    • A consecutive series of 78 eyes underwent combined cyclodialysis and cataract extraction over 26 years.
    • The study analyzed results from 70 eyes with at least six months of follow-up.
    • Glaucoma types included open-angle, angle-closure with peripheral anterior synechias, combined, and secondary to uveitis.

    Findings:

    • Eighty-seven percent of eyes (61/70) achieved a clinically significant reduction in intraocular pressure (IOP).
    • Thirty-nine percent of eyes reached a target IOP of 0-14 mm Hg, and 44% reached 15-20 mm Hg.
    • Lower quadrant cyclodialysis and patient age over 60 were associated with better IOP reduction. Bleeding was a common but unrelated complication.

    Implications:

    • Combining cyclodialysis with cataract extraction is a safe and effective surgical option for managing severe, medically uncontrolled glaucoma.
    • This combined approach offers a viable solution for patients requiring both glaucoma and cataract surgery.
    • The procedure demonstrates good long-term IOP control and is suitable for various glaucoma types.
