Background:

Cluster headaches are a primary headache disorder characterized by severe, unilateral pain.
Orbital exenteration, the removal of orbital contents, is typically performed for severe orbital diseases.

Observation:

A 37-year-old man developed ipsilateral cluster headaches post-orbital exenteration due to a traumatic orbital lesion.
The patient's headache responded well to standard cluster headache treatments.

Findings:

The case and six similar literature reports suggest orbital exenteration can induce cluster headaches.
