Area of Science:

  • Oncology
  • Gastroenterology
  • Surgical Oncology

Background:

  • Gallbladder carcinoma treatment and prognosis have seen advancements, yet no standard treatment protocol is established.
  • The dismal prognosis of gallbladder carcinoma necessitates further research into improved diagnostic and therapeutic strategies.

Purpose of the Study:

  • To review the presentation, staging, treatment, and prognosis of gallbladder cancer patients over three decades at Howard University.
  • To assess improvements in survival rates for gallbladder carcinoma over the past 30 years.

Main Methods:

  • Retrospective analysis of 28 years of gallbladder carcinoma cases at Howard University.
  • Literature review of gallbladder carcinoma diagnostics and treatment over 20 years using Medlars search.
  • Evaluation of the utility of radiological tests for gallbladder cancer diagnosis and staging.

Main Results:

  • Five-year survival rates varied significantly by stage: Stage I (100%), Stage II (51%), Stage III (12%), Stage IV (10%), and Stage V (0%).
  • Abdominal pain was the most common symptom (45%). Diagnostic test sensitivities: Ultrasound (50%), CT (40%), ERCP (75%).
  • Radiotherapy and chemotherapy did not improve survival in this series; cholecystectomy with or without hepatic resection was common for Stages II and III.

Conclusions:

  • Gallbladder carcinoma prognosis is poor, but diagnostic test sensitivity has improved, potentially aiding early detection.
  • Five-fluorouracil combined with radiotherapy shows promise as adjuvant therapy for gallbladder cancer.
  • Aggressive screening of high-risk individuals and early resection of suspicious gallbladder lesions are recommended.
