    Background:

    • Severe penetrating eye injuries often result in complex sequelae.
    • Conventional surgical techniques may be insufficient for treating these challenging cases.

    Observation:

    • Forty-one patients with severe penetrating eye injuries underwent pars plana vitrectomy.
    • The majority of these cases were deemed inoperable by standard methods.

    Findings:

    • A minimum six-month follow-up revealed visual improvement in 13 eyes (32%).
    • Technical success was achieved in six eyes (15%).
    • Intravitreal proliferation of fibrous tissue was the primary cause of surgical failure.

    Implications:

    • Pars plana vitrectomy demonstrates potential, albeit limited, for visual recovery in select cases of severe penetrating eye injuries.
    • Further research is needed to overcome challenges like intravitreal fibrosis and improve outcomes for these complex ocular conditions.
