    Background:

    • Goldmann applanation tonometry is a standard method for measuring intraocular pressure (IOP).
    • Premature ventricular systoles (PVS) are extra heartbeats originating from the ventricles.

    Observation:

    • PVS were identified in six patients during Goldmann applanation tonometry.
    • These cardiac events were associated with rapid decreases in IOP, typically 2-5 mm Hg, returning gradually to baseline.
    • One patient developed atrial fibrillation, and another showed increasing PVS on follow-up ECG.

    Findings:

    • Applanation tonometry may induce or exacerbate premature ventricular systoles.
    • A transient decrease in intraocular pressure is observed concurrently with these cardiac arrhythmias.
    • The cardiac events likely occurred early in the tonometry procedure.

    Implications:

    • Clinicians should be aware of the potential for tonometry to affect cardiac rhythm.
    • Further investigation is warranted to understand the mechanism linking IOP measurement and PVS.
    • This observation may inform patient screening and monitoring during ophthalmic examinations.
