    Summary

    Repository corticosteroid injections can elevate intraocular pressure, with effects lasting longer for less soluble agents. Previous topical corticosteroid use did not predict this response.

    Background:

    • Corticosteroids are frequently used in ophthalmology.
    • Repository corticosteroid injections are a common treatment modality.

    Purpose of the Study:

    • To investigate the relationship between corticosteroid solubility and intraocular pressure elevation.
    • To assess the predictive value of prior topical corticosteroid response on intraocular pressure elevation after repository injections.

    Main Methods:

    • Retrospective analysis of twelve patients (14 eyes) who received repository corticosteroid injections.
    • Correlation of intraocular pressure elevation duration and severity with the solubility of the injected corticosteroid agent.
    • Comparison of response to repository injections with prior topical corticosteroid challenge testing.

    Main Results:

    • Intraocular pressure elevations were observed in all patients following repository corticosteroid injections.
    • The duration and severity of pressure elevations correlated inversely with the solubility of the corticosteroid agent.
    • Two patients with significant pressure elevations post-injection did not show a positive response to prior topical corticosteroid testing.

    Conclusions:

    • The solubility of repository corticosteroids is a critical factor in the duration and severity of induced intraocular pressure elevations.
    • Prior response to topical corticosteroids may not accurately predict the risk of intraocular pressure elevation from repository injections.
