Summary

Patients with essential hypertension and microalbuminuria face higher cardiovascular risks and declining kidney function. Measuring urinary albumin excretion (UAE) can help identify these high-risk hypertensive individuals.

Background:

Purpose of the Study:

  • To investigate the association between microalbuminuria and cardiovascular events in patients with essential hypertension.
  • To assess the impact of microalbuminuria on renal function decline over time.
  • To determine if UAE is a useful marker for cardiovascular risk in hypertension.

Main Methods:

  • A 7-year follow-up study.
  • Inclusion of 141 hypertensive patients, categorized by microalbuminuria status (54 with, 87 without).
  • Monitoring of cardiovascular events and changes in creatinine clearance.

Main Results:

  • Patients with microalbuminuria experienced significantly more cardiovascular events (12 vs. 2).
  • A greater decrease in creatinine clearance was observed in patients with microalbuminuria.
  • Microalbuminuria was linked to abnormal blood pressure variation, elevated LDL-cholesterol, and increased carotid artery thickness.

Conclusions:

  • Hypertensive individuals with microalbuminuria have a higher incidence of cardiovascular events.
  • Microalbuminuria is associated with a more pronounced decline in renal function.
  • Urinary albumin excretion measurement is a valuable tool for assessing cardiovascular risk in essential hypertension.
