Summary

Patients with cystinuria often experience impaired kidney function. Stone-preventive treatments help preserve renal function, highlighting the need for close patient monitoring and early detection of changes using renography.

Area of Science:

  • Nephrology
  • Urology
  • Medical Imaging

Background:

  • Cystinuria is a genetic disorder characterized by recurrent kidney stone formation.
  • Long-term renal functional status in cystinuria patients undergoing stone-preventive treatment is not well-defined.

Purpose of the Study:

  • To evaluate total and separate renal functions in patients with cystinuria.
  • To assess the effectiveness of long-term stone-preventive treatment on renal function.
  • To identify early indicators of renal functional impairment.

Main Methods:

  • Gamma camera renography and glomerular filtration rate (GFR) assessment were performed.
  • Relative renal function (RRF) and separate GFR were analyzed.
  • Patients were monitored over an average of 11.7 years during stone-preventive therapy.

Main Results:

  • A significant percentage of patients exhibited abnormal RRF and reduced GFR at baseline and follow-up.
  • Kidneys with a history of staghorn stones showed significantly lower separate GFR.
  • Despite functional impairment, no cases of terminal renal failure were observed.

Conclusions:

  • Impairment of renal function is prevalent in patients with stone-forming cystinuria.
  • Stone-preventive treatment appears to preserve renal function, necessitating close surveillance.
  • Renography is valuable for early detection of unilateral renal function changes and identifying at-risk patients.
