    Summary

    Children with streptococcal erythema nodosum (EN) show significantly higher serum immunoglobulin E (IgE) levels compared to other causes or healthy controls. This finding highlights a potential biomarker for streptococcal-related EN in pediatric cases.

    Background:

    • Erythema nodosum (EN) is an inflammatory condition affecting subcutaneous fat.
    • The etiology of EN in children can be diverse, including infections and autoimmune conditions.
    • Elevated immunoglobulin E (IgE) levels are associated with allergic and some infectious diseases.

    Purpose of the Study:

    • To investigate serum IgE levels in children diagnosed with erythema nodosum.
    • To compare IgE levels based on the underlying cause of EN, specifically streptococcal and tuberculous origins.
    • To explore the relationship between IgE levels and clinical severity in streptococcal EN.

    Main Methods:

    • Serum samples were collected from 22 children aged 3.5 to 12 years with erythema nodosum.
    • Patients were categorized into etiological groups: streptococcal, tuberculous, and undetermined.
    • Serum IgE levels were quantified using the radioimmunosorbent technique.

    Main Results:

    • Children with streptococcal EN exhibited significantly elevated mean serum IgE levels (991 units/ml).
    • Mean IgE levels in tuberculous EN (97 units/ml) and undetermined etiology groups (68 units/ml) were comparable to healthy controls (60 units/ml).
    • Increased IgE levels in individual streptococcal EN cases did not correlate with clinical manifestation severity.

    Conclusions:

    • Elevated serum IgE levels may serve as a potential indicator for streptococcal-associated erythema nodosum in children.
    • Further research is warranted to elucidate the role of IgE in the pathogenesis of streptococcal EN.
    • IgE levels alone do not appear to predict disease severity in pediatric streptococcal EN.
