    Background:

    • Ocular manifestations of sarcoidosis can significantly impact vision.
    • Understanding the histopathological basis of these changes is crucial for diagnosis and management.

    Observation:

    • A patient with known sarcoidosis presented with typical funduscopic changes.
    • Ophthalmoscopic observations included focal retinal exudates, perivascular "candle wax drippings," preretinal/intravitreal nodules, and tumefactions on the retina and optic nerve head.

    Findings:

    • Histopathological examination of the eye confirmed these ophthalmoscopic findings.
    • The observed changes were attributed to nodular proliferation of epithelioid cells originating from retinal and optic nerve head vasculature.
    • Subpigment epithelial granulomas were present, but granulomas were absent in the choroid and other uveal tissues.

    Implications:

    • This study provides a direct correlation between in vivo ophthalmoscopic findings and ex vivo histopathology in ocular sarcoidosis.
    • It clarifies the cellular origin of characteristic sarcoid lesions within the retina and optic nerve head.
    • The findings enhance our understanding of the pathobiology of ocular sarcoidosis.
