    Background:

    • Right heart failure (RHF) typically involves elevated right atrial pressure.
    • Pulmonary venous hypertension (PVH) is a common cause of RHF.
    • Systemic venous pressure elevation can occur in advanced RHF.

    Observation:

    • Two adult patients presented with pure RHF and chronically elevated systemic venous pressure.
    • Neither patient exhibited signs of pulmonary venous hypertension.
    • Both patients displayed unusual septal lines on diagnostic imaging.

    Findings:

    • The presence of septal lines without PVH suggests an alternative mechanism for RHF.
    • Impaired centripetal lymph flow in the lungs is proposed as the cause.
    • Obstruction of thoracic duct drainage by elevated systemic venous pressure is hypothesized.

    Implications:

    • This finding may redefine understanding of RHF pathophysiology.
    • It highlights a potential link between lymphatic dysfunction and heart failure.
    • Further research into thoracic duct function in RHF is warranted.
