    Summary

    Posterior cervical fusion using rib grafts effectively treated pediatric cervical spine injuries and congenital odontoid absence. Long-term follow-up showed stable, supple necks and no neurological deficits in most patients.

    Background:

    • Cervical spine instability in children poses significant challenges.
    • Congenital absence of the odontoid and traumatic injuries can lead to severe neurological compromise.

    Observation:

    • Six pediatric cases of posterior cervical fusion utilizing autologous rib grafts were analyzed.
    • Procedures involved three-level (C1-3) or four-level (C1-4) fusions.
    • Patients had either traumatic cervical spine injuries or congenital odontoid absence.

    Findings:

    • Five of six patients demonstrated excellent outcomes after 5-13 years of follow-up.
    • All successfully treated children achieved a stable, supple neck without neurological deficits.
    • One patient expired due to unrelated causes post-operatively.

    Implications:

    • Posterior cervical fusion with rib grafts is a viable and effective surgical option for pediatric cervical instability.
    • This technique offers durable stability and functional recovery in young patients.
    • Further research can explore long-term outcomes and refine surgical techniques.
