Summary
A rare parasellar dermoid cyst caused sudden neurological deficit and arterial stenosis. Surgical removal led to full recovery, though cerebral infarction was confirmed post-operatively.

Background:

- Neuroimaging revealed a right supra- and parasellar dermoid cyst causing cerebral arterial stenosis.
Findings:
- Surgical excision via a right subfrontopterional approach was successful.
- Arterial stenosis is attributed to an inflammatory reaction of basal vessels.
Implications:
- This case represents the first reported instance of a parasellar dermoid cyst presenting with neurological deficits and arterial stenosis.
- Complete cyst removal resulted in full patient recovery, despite confirmed postoperative cerebral infarction.
