Soluble adhesion molecules like sICAM-1, sVCAM-1, and sE-selectin are elevated in active polyarteritis nodosa (PAN), indicating immune and endothelial activation. Levels may remain abnormal even in remission, suggesting persistent inflammation.

Background:

  • Systemic polyarteritis nodosa (PAN) is a severe systemic vasculitis.
  • The role of soluble adhesion molecules in reflecting PAN disease activity is not fully understood.

Purpose of the Study:

  • To investigate whether circulating soluble adhesion molecule levels correlate with disease activity in patients with systemic PAN.
  • To assess changes in these molecules during treatment and remission.

Main Methods:

  • Sandwich ELISA was used to measure soluble intercellular adhesion molecule 1 (sICAM-1), vascular cell adhesion molecule 1 (sVCAM-1), E-selectin, L-selectin, and P-selectin.
  • Samples were analyzed from 22 active PAN patients, 13 during follow-up, and 13 healthy controls.

  • Active PAN patients showed significantly higher levels of sICAM-1, sVCAM-1, and sE-selectin compared to controls.
  • sL-selectin levels were lower in active PAN patients.
  • sICAM-1 transiently increased post-treatment initiation, while sVCAM-1 decreased towards normal levels in remission.
  • sE-selectin remained elevated during follow-up.

Conclusions:

  • Elevated sICAM-1, sVCAM-1, and sE-selectin in active PAN suggest immune and endothelial cell stimulation.
  • Persistent abnormal soluble adhesion molecule levels in remission may indicate ongoing subclinical inflammation.
