Summary

Cyclosporine A (CyA) can cause cortical blindness in liver transplant patients. This case shows that blindness may persist even after stopping CyA therapy, unlike typical outcomes.

Area of Science:

  • Neurology
  • Transplantation Medicine
  • Ophthalmology

Background:

  • Cyclosporine A (CyA) is an immunosuppressant crucial for preventing organ rejection in liver transplant recipients.
  • Cortical blindness is a rare but recognized adverse effect of CyA therapy.
  • Neurological symptoms typically resolve upon discontinuation of CyA.

Observation:

  • This report details a liver transplant patient who developed CyA-related cortical blindness.
  • Unlike expected outcomes, the patient experienced only partial neurological improvement after CyA withdrawal.
  • Cortical blindness persisted for over a year despite cessation of CyA treatment.

Findings:

  • The study describes the long-term persistence of cortical blindness following CyA discontinuation in a liver transplant recipient.

Related Experiment Videos

  • Cerebral magnetic resonance imaging (MRI) findings illustrate the evolution of CyA-induced leukoencephalopathy over time.
  • This case highlights a potentially irreversible form of CyA neurotoxicity.
  • Implications:

    • Clinicians should be aware of the possibility of persistent cortical blindness in liver transplant patients on CyA.
    • Further research is needed to understand the mechanisms behind prolonged CyA neurotoxicity.
    • Monitoring neurological function and considering alternative immunosuppressive strategies may be warranted in susceptible patients.
