Background:

  • Biliary complications are a known risk in orthotopic liver transplantation (OLT).
  • Technical issues and arterial supply problems are commonly cited causes.
  • The role of anatomical variations in donor extrahepatic bile ducts remains underreported.

Observation:

  • A 15.4% incidence of biliary complications was observed in 357 OLTs.
  • Anomalous donor extrahepatic ducts were identified in 2.8% of cases (10/357).
  • In 7 of these 10 cases, anomalies were recognized intraoperatively before reconstruction.

Findings:

  • Technical complications occurred in 1 case with intraoperatively identified anomalies.
  • Serious postoperative complications arose in 3 cases where anomalies were missed initially.
