Summary

Obstructive sleep apnea can cause secondary polycythemia, a condition of high red blood cells. Diagnosing and treating sleep apnea with CPAP improved patients and resolved polycythemia.

Background:

  • Obstructive sleep apnea (OSA) is a common disorder characterized by repeated upper airway collapse during sleep.
  • Secondary polycythemia, an elevated red blood cell count, can have various underlying causes.
  • Misdiagnosis of polycythemia can lead to prolonged and ineffective treatments.

Observation:

  • Two patients with severe hypoxemic polycythemia were initially treated with phlebotomy for years without a definitive diagnosis.
  • Both patients were eventually diagnosed with obstructive sleep apnea.

Findings:

  • Introduction of Continuous Positive Airway Pressure (CPAP) therapy led to significant clinical improvement in both patients.
  • Polycythemia regressed in both patients following the initiation of CPAP treatment for OSA.

Implications:

  • Obstructive sleep apnea should be considered in the differential diagnosis of secondary polycythemia.
  • Effective management of OSA can resolve associated hematological abnormalities like polycythemia.
  • This highlights the systemic impact of sleep-disordered breathing and the importance of accurate diagnosis.
