    Nephropexy surgery for nephroptosis (kidney prolapse) showed high success rates, with most patients experiencing relief or a cure. This suggests reconsidering nephropexy in urologic surgery.

    Background:

    • Nephropexy was historically used to treat nephroptosis (kidney prolapse).
    • The efficacy of nephropexy for primary nephroptosis has been debated.
    • Long-term outcomes of nephropexy for nephroptosis are not well-documented.

    Purpose of the Study:

    • To evaluate the long-term outcomes of nephropexy for nephroptosis.
    • To determine the success rates of nephropexy in patients with nephroptosis.
    • To assess factors influencing the success of nephropexy.

    Main Methods:

    • Retrospective review of 21 patients who underwent nephropexy for nephroptosis between 1940 and 1974 at Mayo Clinic.
    • Analysis of follow-up data, including patient-reported symptoms and cure rates.
    • Comparison of outcomes based on surgical fixation methods, postoperative kidney position, and presence of psychologic disorders.

    Main Results:

    • Of 16 patients with long-term follow-up, 14 (87.5%) were cured and 2 (12.5%) had partial relief.
    • Two additional patients with one-year follow-up were asymptomatic.
    • Surgical fixation methods, postoperative kidney position, and psychologic disorders did not significantly impact outcomes.
    • Renography may be beneficial for symptom assessment.

    Conclusions:

    • Nephropexy for nephroptosis demonstrates a high success rate in this patient series.
    • The procedure's effectiveness is not dependent on specific fixation techniques or postoperative kidney mobility.
    • While not advocating a return to its historical popularity, nephropexy warrants consideration in contemporary urologic surgery for select cases.
