Macrophages initiate rheumatic fever inflammation by secreting interleukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-α), preceding lymphocyte involvement. Interleukin-2 (IL-2) appears later, supporting proposed developmental stages of Aschoff nodules.
