Summary

Inhaled nitric oxide (NO) effectively treated low cardiac output syndrome (LOS) in a child after Fontan operation. NO rapidly reduced pulmonary artery pressure and improved hemodynamics when other treatments failed.

Background:

  • The Fontan operation is increasingly indicated, but some patients develop low cardiac output syndrome (LOS) postoperatively.
  • Increased pulmonary vascular resistance (PVR) is a significant contributor to LOS after Fontan procedures.
  • Conventional treatments for LOS can be insufficient in complex cases.

Observation:

  • A 6-year-old boy with a complex cardiac anomaly experienced refractory LOS after a Fontan operation.
  • Standard treatments failed to improve his hemodynamic status during respirator weaning.
  • Inhaled nitric oxide (NO) was administered at a concentration of 6ppm.

Findings:

  • Inhaled NO led to a rapid decrease in pulmonary artery pressure.
  • Hemodynamic parameters significantly improved following NO administration.
