Summary

The intravascular oxygenator (IVOX) offers a simpler alternative to ECMO for gas exchange without affecting hemodynamics. Further improvements are expected to expand its use in acute respiratory failure patients.

Area of Science:

  • Biomedical Engineering
  • Respiratory Physiology
  • Critical Care Medicine

Background:

  • Extracorporeal membrane oxygenation (ECMO) is effective but complex.
  • A simpler intracorporeal device for gas exchange is needed.
  • The intravascular oxygenator (IVOX) aims to address this need.

Purpose of the Study:

  • To describe the IVOX device and its application.
  • To evaluate its current gas exchange capabilities.
  • To discuss its potential for managing acute respiratory failure.

Main Methods:

  • The IVOX is a hollow fiber membrane oxygenator placed in the vena cavae.
  • It facilitates gas exchange without a blood pump.
  • Clinical trials involved 165 patients across 31 centers.

Main Results:

  • IVOX provides 1/4 to 1/3 of the metabolic requirement for gas exchange.
  • It does not impact hemodynamics or body temperature.
  • Mild permissive hypercapnia is recommended for enhanced CO2 removal.

Conclusions:

  • IVOX is a simple, non-hemodynamic impacting device for gas exchange.
  • Current limitations in gas transfer rate necessitate specific patient management strategies.
  • Future design and utilization improvements are anticipated to broaden IVOX applications.
