Background:

  • Hepatocellular carcinoma (HCC) poses significant treatment challenges.
  • Percutaneous ethanol injection (PEI) is an alternative therapy for unresectable HCC.

Purpose of the Study:

  • To evaluate the long-term efficacy of single-session PEI under general anesthesia for HCC.
  • To identify contraindications and risk factors associated with PEI in HCC patients.

Main Methods:

  • A cohort of 100 HCC patients received single-session PEI from 1991-1996.
  • Patients were categorized into three groups based on tumor characteristics and disease stage (A, B, C).
  • Tumor necrosis, complications, and survival rates were assessed post-procedure.

Main Results:

  • Complete tumor necrosis was achieved in 58% of encapsulated tumors and >70% of infiltrating lesions.
  • 1, 2, and 3-year survival rates varied by group, with Group A (encapsulated) showing the highest survival.
  • Observed complications included one death and four major adverse events; risk factors were identified.

  • Single-session PEI is an efficacious treatment for hepatocellular carcinoma.
  • Contraindications include superficial lesions with coagulation defects, severe portal/pulmonary hypertension, bleeding varices, cardiac ischemia, and advanced cirrhosis.
