This study introduces the entirely extraperitoneal endoscopic (EE) radical retropubic prostatectomy (RRP) for prostate cancer. Initial results show successful prostate removal and reconstruction, indicating a promising new minimally invasive surgical option.

Background:

Radical retropubic prostatectomy (RRP) is a standard treatment for prostate cancer.
Advancements in laparoscopic techniques offer potential for less invasive surgical approaches.

Observation:

A novel entirely extraperitoneal endoscopic (EE) radical retropubic prostatectomy (EERRP) approach was performed on a 62-year-old male patient with Gleason score 7 prostate adenocarcinoma.
The procedure involved endoscopic removal of the prostate and seminal vesicles, followed by vesicourethral anastomosis.

Findings:

Complete endoscopic en bloc resection of the prostate and seminal vesicles was achieved.
A successful, watertight vesicourethral anastomosis was reconstructed.
The surgery resulted in a 5-hour 45-minute operative time, 600 cc blood loss, and a 2.5-day hospital stay.
