    Background:

    • Progressive multifocal leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the central nervous system (CNS).
    • PML is caused by the JC virus (JCV) in immunocompromised individuals.
    • Understanding the immunologic underpinnings of PML is crucial for diagnosis and management.

    Observation:

    • This study presents two cases of PML with multiple concurrent CNS infections.
    • Both patients exhibited impaired cell-mediated immunity with normal immunoglobulin levels.
    • One patient experienced a notable 8-month neurologic remission and showed a positive brain scan linked to a large frontal demyelinating lesion.

    Findings:

    • Defective cellular immunity, rather than humoral immunity, appears to be a key factor predisposing to PML.
