Duplication of the urethra

Summary

This study on urethral duplication in ten patients found the ventral channel is typically the more functional urethra. Complete evaluation requires both voiding cystourethrography and retrograde urethrography for these complex congenital anomalies.

Background:

Urethral duplication is a rare congenital anomaly.
Understanding its variations is crucial for effective management.
Previous classifications may not fully capture anatomical and functional aspects.

Observation:

Ten patients with patent urethral duplication were evaluated.
Nine presented with two independent urethral channels originating from the bladder or posterior urethra.
The ventral channel consistently demonstrated greater functionality, irrespective of meatal location.

Findings:

Voiding cystourethrography and retrograde urethrography are essential for complete assessment.
Dorsal channels in an epispadiac position were associated with abnormal widening of the symphysis pubis.
A new, functionally and anatomically accurate classification system for urethral duplication is proposed.

Implications:

The proposed classification aids in precise diagnosis and surgical planning.
Understanding the functional significance of each urethral channel optimizes treatment outcomes.
Further research into the embryology of urethral duplication can refine understanding and prevention strategies.
