Assessment of marital relationships of hemodialysis patients
Insights
Successful hemodialysis requires psychological adjustment. This study examined marital interactions and conflicts in patients undergoing hemodialysis, revealing their impact on the therapy
Area of Science:
- Nephrology
- Psychology
- Sociology
Background:
- Somatic issues of uremia and dialysis are well-documented.
- Psychological adjustment is crucial for successful hemodialysis.
- Previous research has focused on individual patient or spouse adjustment, neglecting marital dynamics.
Purpose of the Study:
- To systematically evaluate patterns of marital interactions and conflicts in patients undergoing hemodialysis.
- To understand the role of the marital relationship in the psychological adjustment to hemodialysis therapy.
Main Methods:
- A study involving 17 medically stable hemodialysis patients and their spouses.
- Standardized psychiatric evaluations for mood and cognition in both patients and spouses.
- A detailed marital questionnaire completed independently by patients and spouses to assess relationship aspects.
Main Results:
- Identified specific patterns of marital interactions and conflicts.
- Assessed the influence of these marital factors on patient psychological adjustment.
- Provided insights into the fundamental aspects of the marital relationship in the context of long-term hemodialysis.
Conclusions:
- Marital interactions and conflicts are significant factors in the psychological adjustment to hemodialysis.
- A comprehensive understanding of marital dynamics is essential for supporting patients on hemodialysis.
- Future interventions should consider the couple's relationship for improved patient outcomes.
Abstract:
In addition to the somatic problems associated with uremia and dialysis it is apparent that successful hemodialysis is dependent upon psychological adjustment to this type of therapy. While previous studies have focused on the patient or spouse as individuals, patterns of marital interactions and conflicts have not been systematically evaluated. These factors ere examined in 17 medically stable patients maintained on hemodialysis for a mean duration of 22 +/- 4 SE months. Standardized pssychiatric evaluation forms ere used to examine mood and cognition disturbances in all patients and spouses, and a deailed marital questionnaire, which each patient and spouse filled out independently, was used to evaluate fundamental aspects of the marital relationship...
