Summary

Patients with polygenic hypercholesterolemia (PH) carrying the apolipoprotein E4 (apoE4) genotype show higher cholesterol synthesis rates. This finding may explain varied responses to statin therapy in PH patients.

Background:

  • Polygenic hypercholesterolemia (PH) is a common form of high cholesterol.
  • Apolipoprotein E (apoE) genotype influences lipid metabolism.
  • Interindividual variability in cholesterol synthesis and treatment response exists.

Purpose of the Study:

  • To investigate cholesterol synthesis rates in PH patients with different apoE genotypes.
  • To determine if the apoE4 allele is associated with increased cholesterogenesis in PH.
  • To explore the implications for statin therapy response.

Main Methods:

  • Studied 22 normal-weight PH patients (11 apoE3/4, 11 apoE3/3).
  • Patients maintained a standardized low-fat, low-cholesterol diet for 3 months.

  • Measured daily urinary mevalonic acid (MVA) excretion as an index of cholesterol synthesis.
  • Main Results:

    • Urinary MVA excretion was significantly higher in apoE3/4 patients (2.52 ± 0.8 µmol/24h) compared to apoE3/3 patients (1.38 ± 0.6 µmol/24h) (P < .001).
    • This is the first evidence of elevated cholesterogenesis in PH patients with the apoE4 allele under a controlled diet.
    • Groups were comparable in age, BMI, and baseline cholesterol levels.

    Conclusions:

    • The apoE4 allele is associated with increased whole-body cholesterol synthesis in PH patients.
    • Higher cholesterogenesis in apoE4 carriers may partly explain differences in statin response.
    • Further research into apoE genotype and cholesterol metabolism is warranted.
