Summary

Patent urachus is a rare congenital anomaly where the allantois remnant persists, potentially mimicking an omphalocele on ultrasound. Surgical repair offers a favorable fetal prognosis with fewer associated abnormalities.

Area of Science:

  • Embryology
  • Fetal Medicine
  • Pediatric Surgery

Background:

  • Patent urachus arises from the failure of the allantois to obliterate during embryonic development.
  • This condition can present as an abdominal wall defect, often mistaken for an omphalocele on prenatal ultrasound.

Observation:

  • A case of patent urachus diagnosed in a primigravida at 19 weeks' gestation is presented.
  • Ultrasound revealed a cystic mass at the umbilical cord insertion, appearing contiguous with the urinary bladder.
  • The mass was initially suspected to be an omphalocele due to its abdominal wall origin.

Findings:

  • Serial sonography tracked the abdominal wall mass throughout gestation.
  • The infant was delivered via Cesarean section and underwent successful surgical repair of the patent urachus and abdominal wall reconstruction.

Related Experiment Videos

  • Patent urachus can result in a urinary fistula, mimicking omphalocele sonographically.
  • Implications:

    • Patent urachus has a significantly better fetal prognosis compared to omphalocele due to a lower incidence of associated anomalies.
    • Accurate prenatal diagnosis and differentiation from omphalocele are crucial for appropriate management and counseling.
    • Surgical intervention for patent urachus leads to successful outcomes and reconstruction.
