    Diabetic macular edema in patients with long-standing diabetes can occur without significant retinopathy. Laser treatment improved edema locally but did not restore visual acuity.

    Background:

    • Diabetic macular edema (DME) is a common complication of diabetes.
    • Patients with long-standing diabetes may present with atypical DME.
    • This study investigates DME in patients with minimal background diabetic retinopathy.

    Observation:

    • Three patients with long-standing diabetes exhibited reduced visual acuity and bilateral symmetrical cystoid macular edema.
    • Minimal signs of background diabetic retinopathy were observed.
    • Fluorescein angiography revealed diffuse macular capillary leakage with preserved perfusion.

    Findings:

    • Argon laser photocoagulation treated localized cystoid macular edema effectively.
    • Untreated foveal and perifoveal areas showed no resolution of edema.
    • Visual acuity did not improve in any of the three patients post-treatment.

    Implications:

    • Localized laser treatment may not be sufficient for diffuse diabetic macular edema.
    • Further research is needed to understand and treat DME in atypical diabetic presentations.
    • Visual acuity outcomes suggest limitations of current treatment modalities for this specific patient group.
