Summary

Surgical drainage effectively treated a rare case of idiopathic suppurative pylephlebitis with extensive portal vein thrombosis. This intervention rapidly reduced fever and resolved symptoms, offering an alternative to conservative antibiotic therapy.

Background:

  • Idiopathic suppurative pylephlebitis is a rare and potentially life-threatening condition.
  • Conservative management with antibiotics and urokinase has been reported, but surgical intervention is less common.

Observation:

  • An 18-year-old male presented with fever and abdominal pain, exhibiting widespread thrombosis of the portal vein.
  • Diagnostic imaging, including ultrasonography and computed tomography, confirmed extensive venous involvement.

Findings:

  • Emergency laparotomy revealed massive pus accumulation within the occluded superior mesenteric vein.
  • Surgical drainage of the infected thrombus led to a rapid and significant decrease in the patient's fever and clinical improvement.
