Transient hypothyroidism after delivery in autoimmune thyroiditis

N Amino, K Miyai, T Onishi

The Journal of Clinical Endocrinology and Metabolism

    Summary

Postpartum autoimmune thyroiditis can cause temporary hypothyroidism and goiter in women. Thyroid function typically normalizes within months without treatment, suggesting pregnancy-induced changes.

    Area of Science:

    • Endocrinology
    • Immunology
    • Reproductive Medicine

    Background:

    • Autoimmune thyroiditis is a common cause of thyroid dysfunction.
    • Pregnancy and delivery are known to influence autoimmune conditions.
    • Postpartum thyroid dysfunction can manifest in various forms.

    Purpose of the Study:

    • To investigate the occurrence and natural course of primary hypothyroidism following delivery in women with autoimmune thyroiditis.
    • To analyze changes in thyroid hormone levels, thyrotropin, and antibody titers during the postpartum period.
    • To determine if pregnancy and delivery trigger transient thyroid dysfunction in autoimmune thyroiditis.

    Main Methods:

    • Observation of 6 women with autoimmune thyroiditis during the postpartum period.

    Related Experiment Videos

  • Monitoring of thyroid hormone levels (thyroxine iodine, triiodothyronine), T3 resin uptake, and serum thyrotropin.
  • Measurement of anti-thyroglobulin and anti-thyroid microsomal antibody titers.
  • Main Results:

    • Transient primary hypothyroidism and diffuse goiter observed 1-3 months postpartum.
    • Lowest thyroid hormone levels and highest thyrotropin levels noted at 3-6 months postpartum.
    • Thyroid function normalized spontaneously within 6-9 months postpartum; anti-thyroid microsomal antibodies were positive in all cases.

    Conclusions:

    • Pregnancy and delivery may induce transient hypothyroidism and goiter in women with autoimmune thyroiditis.
    • Spontaneous recovery of thyroid function is common in these cases.
    • Elevated anti-thyroid antibody titers correlate with the period of hypothyroidism.
