Background:

  • Spinal epidural hematoma (SSEH) is an infrequent cause of acute nontraumatic myelopathy.
  • Prompt diagnosis and surgical intervention are critical for managing spinal cord compression.

Observation:

  • A 14-year-old male presented with sudden-onset tetraplegia upon waking.
  • Magnetic resonance imaging confirmed the diagnosis of spinal epidural hematoma.

Findings:

  • Despite surgical decompression within 9 hours of symptom onset, the patient experienced no neurological recovery and remained tetraplegic.
  • No underlying cause for the spontaneous bleeding was identified.

Implications:

  • This case highlights the challenges in predicting outcomes for SSEH, even with timely treatment.
