Summary

Acute appendicitis (AA) can lead to aerobic urinary tract infections (UTIs). Bacteria from the appendix may spread, causing UTIs in patients undergoing appendectomy.

Background:

  • Urinary tract infections (UTIs) associated with acute appendicitis (AA) are infrequently reported.
  • Understanding the link between AA and UTIs is crucial for patient management.

Purpose of the Study:

  • To investigate the effect of acute appendicitis on aerobic urinary tract infections.
  • To identify bacterial strains common to both the appendix and urinary tract in AA patients.

Main Methods:

  • Collected appendiceal tissue and urine samples from 66 patients with confirmed AA undergoing appendectomy.
  • Utilized control groups (inguinal hernioplasty, varicocele repair) for comparison.
  • Performed urinalysis, aerobic cultures, and E. coli "O" serogrouping on samples collected pre- and post-operatively.

Main Results:

  • Abnormal urinalysis was observed in 48% of AA patients pre-operatively, decreasing to 12% post-operatively.
  • Aerobic bacteria were isolated from urine in 32% pre-operatively and 24% post-operatively.
  • Identical aerobic strains (E. coli, Streptococcus faecalis, Corynebacterium) were found in the appendix, mesenteriolum, and urine in 11% of AA patients, with 4 developing UTIs.

Conclusions:

  • Bacteria from the inflamed appendix can spread to the retroperitoneal space and potentially infect the urinary tract.
  • This can result in symptomatic or asymptomatic urinary tract infections in patients with acute appendicitis.
  • Early detection and management of potential UTIs in AA patients are important.
