This case study highlights a rare instance of rapidly progressive glomerulonephritis (RPGN) co-occurring with diabetic nephropathy in an elderly woman, emphasizing the diagnostic challenges and complex renal pathology involved.

Background:

Diabetic nephropathy is a common complication of diabetes mellitus.
Rapidly progressive glomerulonephritis (RPGN) is a syndrome of rapid loss of kidney function.
Coexistence of these two conditions is rare and presents diagnostic challenges.

Observation:

A 68-year-old woman with non-insulin-dependent diabetes mellitus and diabetic triopathy experienced rapid renal function decline.
Urinary analysis revealed red blood cells and casts, suggesting an additional renal pathology.
Renal biopsy showed crescent formation, indicative of pauci-immune RPGN.

Findings:

The patient presented with a rare combination of diabetic nephropathy and pauci-immune RPGN.
Renal biopsy confirmed crescentic glomerulonephritis superimposed on diabetic glomerulosclerosis.

Steroid pulse therapy provided only transient improvement in renal function.

Implications:

This case underscores the importance of considering superimposed glomerulonephritis in diabetic patients with rapidly declining renal function.
Early diagnosis and appropriate management are crucial for potentially altering the disease course.
Further research is needed to understand the mechanisms and optimal treatment strategies for such complex cases.
