This case study describes a rare alpha-fetoprotein (AFP)-producing rectal carcinoma. Such tumors are highly malignant, evidenced by early liver metastasis and rapid progression in this patient.

Background:

  • Rectal carcinoma is a common malignancy.
  • Alpha-fetoprotein (AFP) is a tumor marker typically associated with liver cancer and germ cell tumors.
  • AFP-producing tumors outside of the liver and germ cells are rare.

Observation:

  • A 71-year-old male with rectal carcinoma developed liver metastases and significantly elevated AFP levels (220,000 ng/ml) six months post-rectectomy.
  • Histological examination revealed well-differentiated adenocarcinoma with areas of medullary and trabecular proliferation.
  • Immunohistochemistry confirmed AFP localization in the trabecular areas of the rectal tumor.

Findings:

  • The AFP identified in the rectal carcinoma exhibited lectin binding characteristics between hepatic and yolk sac types, suggesting an intestinal origin.
