Summary

This study investigated electrophysiologic findings in a patient with multiple evanescent white dot syndrome (MEWDS). Results indicate wider retinal dysfunction than previously detected, suggesting optic nerve involvement in MEWDS.

Background:

  • Multiple evanescent white dot syndrome (MEWDS) is an idiopathic inflammatory condition affecting the outer retina.
  • Diagnosis typically relies on fundus examination, visual acuity, and fluorescein angiography.
  • Electrophysiologic testing can provide objective measures of retinal function.

Observation:

  • A 19-year-old female presented with decreased visual acuity and characteristic fundus findings of MEWDS.
  • Fundus examination revealed white dots with hyperfluorescence in the affected eye.
  • Standard electroretinography (ERG) and visual evoked cortical potentials (VEPs) were performed.

Findings:

  • Flicker ERG amplitude was reduced, while single-flash ERG was normal.

  • Multifocal ERG (mfERG) amplitudes were significantly reduced in scotoma areas and moderately reduced elsewhere in the central field.
