Summary

Bilateral forearm plethysmography, measuring forearm blood flow (FBF) ratios, offers greater reproducibility than unilateral measurements at rest and during vasoactive substance infusions. Forearm vascular resistance (FVR) shows a similar reproducibility advantage.

Background:

  • Strain-gauge plethysmography is used to measure forearm blood flow (FBF) changes during vasoactive substance infusions.
  • Bilateral measurements, expressed as FBF ratios, are common, but their reproducibility is not well-established.
  • Unilateral measurements and forearm vascular resistance (FVR) are alternative methods used by some researchers.

Purpose of the Study:

  • To assess the intra-subject variability of bilateral FBF measurements (FBF ratios) at rest, post-exercise, and during vasoconstrictor infusions.
  • To compare the reproducibility of bilateral plethysmography with unilateral plethysmography.
  • To evaluate the reproducibility of forearm vascular resistance (FVR) measurements, both unilateral and bilateral.

  • Study 1: Nine healthy subjects underwent FBF measurements at rest and after unilateral forearm exercise on three separate days.
  • Study 2: Five healthy subjects had FBF measured during incremental infusions of noradrenaline and angiotensin II on two separate days.
  • Intra-subject variability was calculated using coefficients of variation (CV) and differences between responses on different days.

Main Results:

  • At rest, FBF ratios showed lower intra-subject variability (CV 19%) compared to unilateral FBF measurements (CV 31-39%).
  • Following exercise, unilateral measurements were initially more reproducible, but FBF ratios re-established better reproducibility after 20 minutes.
  • During vasoconstrictor infusions, FBF ratios demonstrated better reproducibility (angiotensin II: 14% vs 18%; noradrenaline: 16% vs 27%) than unilateral FBF.

Conclusions:

  • Bilateral forearm plethysmography, using FBF ratios, is more reproducible than unilateral measurements for assessing FBF at rest and in response to vasoactive infusions.
  • Forearm vascular resistance (FVR) may offer a slight reproducibility advantage.
  • Unilateral measurements can lead to misleading results due to non-experimental stimuli; bilateral FBF ratios are recommended for intra-arterial infusion studies.
