    Summary

    Severe psoriasis patients treated with methotrexate showed no signs of kidney damage in biopsies. This study investigated the renal safety of long-term methotrexate use in psoriatic individuals.

    Background:

    • Psoriasis is a chronic inflammatory skin condition.
    • Methotrexate is a common systemic treatment for severe psoriasis.
    • Concerns exist regarding potential methotrexate-induced kidney toxicity.

    Purpose of the Study:

    • To evaluate the renal safety of long-term methotrexate therapy in patients with severe psoriasis.
    • To assess for histological evidence of kidney damage in psoriatic patients undergoing treatment.

    Main Methods:

    • Retrospective analysis of kidney biopsies from eight psoriatic patients.
    • Concurrent review of 429 liver biopsies from the same patient cohort.
    • Correlation of biopsy findings with patient history, including methotrexate dosage and duration.

    Main Results:

    • No renal damage was observed in any of the eight kidney biopsies.
    • All patients had severe psoriasis; six were on methotrexate for 1-6 years.
    • Total methotrexate dosages ranged from 660 to 1757 g.

    Conclusions:

    • Long-term methotrexate treatment for severe psoriasis did not appear to cause detectable kidney damage in this cohort.
    • Kidney biopsies in psoriatic patients treated with methotrexate showed no histological signs of toxicity.
    • Further research may be warranted to confirm these findings in larger populations.
