Summary

Continent urinary diversion can develop calculi, a frequent complication. Extra corporeal wave shock lithotripsy (ESWL) successfully treated pouch calculi in two patients, proving to be a safe and effective non-invasive option.

Area of Science:

  • Urology
  • Surgical Innovation
  • Nephrolithiasis Management

Background:

  • Continent urinary diversion, often an ileal conduit like the Indiana pouch, replaces the bladder after cystectomy.
  • Calculi formation within the urinary diversion pouch is a recognized and frequent late complication.

Observation:

  • Two patients presented with calculi in their Indiana pouch, two years post-diversion surgery.
  • These calculi posed a clinical challenge requiring effective treatment.

Findings:

  • Extra corporeal wave shock lithotripsy (ESWL) was successfully employed to treat the pouch calculi in both patients.
  • ESWL demonstrated efficacy in fragmenting and clearing the calculi without compromising pouch function.

Implications:

  • ESWL represents a viable, non-invasive therapeutic option for managing calculi in continent urinary diversions.
  • This technique offers a safe alternative, preserving the continence of the urinary pouch.
