Summary

Insulin resistance is linked to increased lipid oxidation. While individuals with insulin-dependent diabetes mellitus (IDDM) show higher muscle triglyceride (mTG) content, this doesn't correlate with insulin sensitivity in either healthy or diabetic subjects.

Purpose of the Study:

  • To investigate muscle triglyceride (mTG) concentration and its relationship with insulin sensitivity.
  • To compare mTG levels and insulin sensitivity between healthy men and men with insulin-dependent diabetes mellitus (IDDM).

Main Methods:

  • Utilized a 4-hour euglycaemic-hyperinsulinaemic clamp with indirect calorimetry to assess insulin sensitivity.
  • Measured mTG concentration via percutaneous biopsy of the m. vastus lateralis.
  • Included 10 healthy men and 17 men with IDDM.

Main Results:

  • IDDM patients exhibited higher baseline glucose, insulin, HDL cholesterol, and mTG concentrations compared to healthy men.
  • IDDM patients showed reduced insulin-stimulated glucose disposal rates (total, oxidative, and non-oxidative).
  • IDDM patients had higher basal and hyperinsulinaemic lipid oxidation rates. mTG did not correlate with insulin-stimulated glucose disposal, but correlated positively with lipid oxidation in healthy men.

Conclusions:

  • Insulin-dependent diabetes mellitus (IDDM) is associated with elevated intramuscular triglyceride (mTG) content.
  • mTG content does not demonstrate a correlation with insulin sensitivity in either healthy individuals or patients with IDDM.
