Summary

Rare CD5 positive marginal zone B-cell lymphomas (MZBCLs) can present with localized disease, differing from typical widespread presentations. This finding expands understanding of MZBCL phenotypes and clinical behavior.

Background:

  • Marginal zone B-cell lymphomas (MZBCLs) are typically CD5 negative low-grade lymphomas.
  • Rare cases of CD5 positive MZBCL (CD5+ MZBCL) have been reported, often with widespread disease.

Observation:

  • This study presents four new cases of CD5+ MZBCL.
  • Three cases were mucosa-associated lymphoid tissue (MALT) lymphomas (lungs, conjunctiva, uterus).
  • One case was a monocytoid B-cell lymphoma (cervical lymph node).

Findings:

  • All four patients presented with localized disease.
  • Peripheral blood counts were normal, and bone marrow staging showed no involvement in three patients.
  • Southern blot analysis did not reveal bc11 or bc12 rearrangements in analyzed cases.

  • CD5+ MZBCL can exhibit an atypical phenotype.
  • Unlike most reported cases, these CD5+ MZBCLs were clinically localized at presentation.
  • This suggests a subset of CD5+ MZBCL may have a more indolent clinical course.
