Summary

Tranilast, used to reduce prednisolone, caused eosinophilic cystitis in a patient. Symptoms of hematuria and bladder irritation resolved rapidly after discontinuing tranilast.

Background:

  • A 62-year-old male with idiopathic thrombocytopenic purpura was treated with long-term prednisolone.
  • Tranilast was administered for 18 months to reduce the prednisolone dosage.

Observation:

  • The patient presented with macroscopic hematuria and bladder irritative symptoms.
  • Urinalysis showed a significant increase in eosinophils (4 x 10^4/ml).
  • Cystoscopy revealed diffuse bladder mucosal redness; cystogram showed bilateral vesicoureteral reflux.

Findings:

  • Histological examination of bladder biopsies confirmed marked eosinophilic infiltration (1,126/mm2) with minimal mast cells (12/mm2).
  • No history of allergies was reported by the patient.

Implications:

  • This case highlights tranilast as a potential cause of drug-induced eosinophilic cystitis.
  • Prompt resolution of symptoms upon drug cessation underscores the etiological link.
  • Clinicians should consider eosinophilic cystitis in patients presenting with urinary symptoms during tranilast therapy.
