Summary

Hemodialysis triggers inflammatory cytokine gene expression, particularly tumor necrosis factor alpha (TNF alpha), early in treatment. However, this initial gene activity doesn't always translate to increased protein levels later in the dialysis session.

Purpose of the Study:

  • To evaluate the gene transcription of inflammatory cytokines during hemodialysis.
  • To investigate the relationship between hemodialysis membrane types and cytokine gene expression.
  • To compare the early gene transcription of cytokines with later plasma levels of mature cytokines.

Main Methods:

  • Blood and plasma samples were collected from 22 hemodialysis patients at different time points (0, 15, and 180 minutes).
  • RNA was extracted from mononuclear cells, reverse transcribed, and amplified using PCR to measure gene expression of tumor necrosis factor alpha (TNF alpha), interleukin (IL) 1 beta, and IL6.

    • A significant increase in TNF alpha messenger RNA (mRNA) was observed at 15 minutes of hemodialysis in most patients, varying by membrane type.
    • Parallel increases in IL1 beta and IL6 mRNA were detected, with significant correlations between TNF alpha and IL1 beta, and IL1 beta and IL6 gene expression.
    • Increased plasma levels of mature TNF alpha and IL1 beta were found in patients with elevated cytokine gene expression, but early mRNA levels did not predict later protein concentrations.

    Conclusions:

    • Cytokine mRNA transcription is a common early event during hemodialysis.
    • Gene expression does not always lead to protein translation, possibly due to mRNA degradation.
    • Cell-to-cell interaction mechanisms may be crucial for driving cytokine activation to clinical relevance.
