Background:

  • Bile duct stones pose risks, especially in older patients with comorbidities.
  • Standard surgical treatment for bile duct stones carries increased morbidity and mortality in high-risk individuals.

Purpose of the Study:

  • To compare the efficacy and safety of open surgery versus endoscopic sphincterotomy (ES) for bile duct stones in high-risk patients.
  • To evaluate long-term outcomes, including symptom recurrence, for both treatment modalities.

Main Methods:

  • A randomized controlled trial involving 98 high-risk patients (mean age 80 years) with bile duct stones or biliary pancreatitis.
  • Patients were assigned to either open cholecystectomy with bile duct exploration (n=48) or endoscopic sphincterotomy (ES) alone (n=50).

Main Results:

  • Immediate success rates were similar (94% surgery vs. 88% ES), with no significant differences in initial morbidity or mortality.
  • However, endoscopic sphincterotomy (ES) was an independent predictor of recurrent biliary symptoms (OR 6.9), leading to repeat procedures in 70% of affected ES patients.

Conclusions:

  • Open surgery is the preferred definitive treatment for bile duct stones or non-severe biliary pancreatitis in elderly or high-risk patients.
  • Endoscopic sphincterotomy (ES) alone is associated with a higher likelihood of symptom recurrence and need for subsequent interventions.
