    Ruptured intracranial aneurysms can cause brain damage through emboli, not just vasospasm. These emboli, originating from internal carotid artery aneurysms, lead to cerebral infarctions, highlighting a critical cause of aneurysm-related morbidity.

    Background:

    • Cerebral vasospasm is a known complication following subarachnoid hemorrhage from ruptured intracranial aneurysms.
    • This vasospasm is thought to be the primary cause of secondary cerebral ischemia and infarction.

    Observation:

    • Two cases are presented where aneurysms of the internal carotid arteries were identified as the source of emboli.
    • These emboli led to cerebral infarctions in the affected patients.

    Findings:

    • Embolic events originating from intracranial aneurysms can cause cerebral infarction.
    • This mechanism contributes to the morbidity associated with ruptured aneurysms.

    Implications:

    • Embolic phenomena should be considered in the differential diagnosis of neurological deficits after aneurysm rupture.
    • This finding may influence diagnostic and therapeutic strategies for managing intracranial aneurysms.
