Summary

Sentinel lymph node biopsy (SNB) accurately identifies breast cancer metastasis. A negative SNB result may allow women to avoid axillary lymph node dissection (ALND).

Background:

Sentinel lymph node biopsy (SNB) is a potential alternative to axillary lymph node dissection (ALND) for staging breast cancer.
Accurate axillary staging is crucial for effective breast cancer treatment.

Purpose of the Study:

To evaluate the success and accuracy of axillary SNB using isosulfan blue (ISB) and technetium-99 sulfur colloid (TSC).
To compare SNB findings with axillary lymph node dissection (ALND) results in breast cancer patients.

Main Methods:

Forty-two women with T1 or T2 breast cancer underwent SNB followed by ALND.
A mixture of ISB and TSC was injected prior to surgery for SLN identification.
SLN identification utilized a gamma detector and blue dye visualization.

Main Results:

An axillary sentinel lymph node was identified in 90% of cases.
The SLN accurately predicted axillary metastases, with a 100% negative predictive value.
Previous excisional biopsy did not affect SLN localization or predictive value.

Conclusions:

A negative sentinel lymph node biopsy may obviate the need for axillary lymph node dissection in select breast cancer patients.
Further validation in larger studies is warranted to confirm these findings.
