Background:

  • Ice pick-like pain, typically brief and severe, is often associated with the head and face.
  • Extratrigeminal ice pick status, pain outside the trigeminal nerve distribution, has been recently described.

Observation:

  • Two single men presented with distinct neurological conditions: compressive spondylitic cervical myelopathy and tethered cord with intraspinal lipoma.
  • Both individuals reported severe, paroxysmal ice pick-like pains localized to the neck or groin/genitalia, respectively.
  • These pains were consistently precipitated by masturbation and masturbatory orgasms, with continuous background pain in the same areas.

Findings:

  • The cases demonstrate extracephalic (outside the head) ice pick-like pain triggered by sexual activity.
  • The pain occurred in the absence of intracranial lesions, epilepsy, migraine, or orgasmic cephalgia (headache).
  • This suggests a potential link between specific spinal cord conditions and sexual activity-induced pain in extracephalic regions.
