Summary

Ping-pong gaze (PPG) can appear smooth or saccadic. Saccadic PPG in unconscious patients may indicate a lighter state of consciousness and better survival prognosis compared to smooth PPG.

Background:

  • Ping-pong gaze (PPG) involves alternating horizontal eye movements.
  • Previous descriptions suggested PPG waveforms are typically smooth.

Observation:

  • An electrooculographic study examined four unconscious patients with PPG.
  • Waveform analysis revealed variations: one patient showed smooth PPG, while three exhibited saccadic cog-wheeling.

Findings:

  • Smooth PPG waveforms were associated with immediate mortality.
  • Saccadic PPG waveforms correlated with survival in a persistent vegetative state.
  • A transition from smooth to saccadic PPG coincided with clinical improvement in one patient.

Implications:

  • Saccadic PPG may represent a distinct clinical variant of PPG.
