Background:

  • A 10-year-old boy with a left inguinal hernia presented with scrotal redness and tenderness.
  • Initial suspicion of testicular or appendage torsion led to surgical exploration.

Observation:

  • Scrotal exploration revealed purulent fluid and inflammation within the tunica vaginalis.
  • Postoperative persistence of fever and abdominal tenderness, with tenderness over Lanz's point, suggested an intra-abdominal source.

Findings:

  • Abdominal imaging confirmed an appendiceal abscess between the bladder and sacrum.
  • Laparotomy revealed a gangrenous appendix; the inguinal hernia sac was obscured by adhesions.
