    Phenobarbital reduces Adriamycin efficacy by increasing its metabolism in the liver. Concurrent use of phenobarbital and Adriamycin is not recommended for optimal therapeutic outcomes.

    Background:

    • Adriamycin (doxorubicin) disposition is influenced by liver metabolism and biliary excretion.
    • Drugs that enhance liver metabolism may reduce Adriamycin concentrations to suboptimal levels.

    Purpose of the Study:

    • To investigate the effect of phenobarbital, a liver enzyme inducer, on Adriamycin metabolism and efficacy in mice.

    Main Methods:

    • Mice were pretreated with phenobarbital before Adriamycin administration.
    • Plasma and liver concentrations of Adriamycin and its metabolites were measured.
    • Liver microsomal activity and enzyme kinetics were analyzed.
    • Survival rates of tumor-bearing mice treated with Adriamycin and phenobarbital were assessed.

    Main Results:

    • Phenobarbital pretreatment led to faster elimination of Adriamycin from plasma.
    • Increased concentrations of Adriamycin glycones were found in the livers of phenobarbital-treated mice.
    • Phenobarbital-induced liver microsomes showed a more rapid initial rate of aglycone production.
    • Survival rates were significantly lower in phenobarbital-pretreated mice receiving Adriamycin.

    Conclusions:

    • Phenobarbital enhances Adriamycin metabolism via microsomal enzyme induction.
    • Concurrent administration of Adriamycin with liver enzyme-inducing drugs may compromise therapeutic efficacy.
    • Dosage adjustments or avoidance of such drug combinations are suggested for optimal Adriamycin treatment.
